Alternative Test Request Forms need to be submitted at least 3 business days in advance. If you are unable to do so for any reason, submit the form as soon as possible so that we can try our best to accommodate your needs. Instructors requesting makeup exams for non-ODA students can be submitted through this form as well, although students registered with the ODA will receive priority. If a student is over 15 minutes late or misses a scheduled testing time (illness, childcare, transportation, etc.), it will require professor approval before it may be rescheduled in Frisco. It will be the student's responsibility to contact the instructor about rescheduling the exam. It is the instructor's responsibility to approve or deny rescheduling the student's test and send their decision via email to Marlena.Jordan@unt.edu
Failure by instructor to provide exam materials ahead of/at the agreed upon start time may result in the exam requiring rescheduling at a time agreed upon by the student and instructor with no penalty to the student.

For online exams, if the student is unable to access the test due to incorrect password, password not being provided, or exam not unlocking at agreed upon time, the proctor will not be able to administer the exam unless the issue is resolved within 30 minutes of the intended start time. If the issue is unresolved, the exam must be rescheduled to another time/date agreed upon by the student and instructor with no penalty to the student.
