We need the following paperwork filled out by student and parent in order to participate in the day. Please ensure both forms are filled out and 3 documents are uploaded to this form at least 5 days before the event. If you do not fill out your paperwork and we do not confirm your place for your event, you will not be eligible to be on campus as a minor.
